About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health - South team as a Social Worker, where you will be responsible for:

* Engaging and supporting vulnerable young people to access and navigate youth-focused primary health care, including young parents and those impacted by adverse childhood experiences (ACEs).
* Delivering high-quality, trauma-informed case management and psychosocial support aligned with the MYH Model of Care, through both onsite and outreach services.
* Collaborating with families, health professionals, and community partners to improve health outcomes and empower young people in managing their own care.
* Contributing to service development and improvement through evaluation, data collection, research, and implementation of best practice standards and policies.
* Providing clinical supervision and mentoring to students and staff, supporting a culture of professional growth, cultural sensitivity, and continuous learning.

About You

To be successful in this role, you will have:

* Appropriate social work degree or equivalent qualification eligible for full membership with the Australian Association of Social Workers (AASW).
* Strong interpersonal and written communication skills, able to engage effectively with diverse populations and collaborate within multidisciplinary teams.
* Experience providing assessment, case planning, psychosocial support, referral, advocacy, and follow-up for young people with complex needs.
* Solid understanding of adolescent psychosocial development, youth health issues, mandatory reporting, information sharing, duty of care, and consumer rights.
* Awareness of social determinants affecting vulnerable groups, including young people under guardianship and those from Aboriginal and Torres Strait Islander backgrounds, to ensure culturally appropriate health services.

About MYH:

* MYH works on prevention, early intervention and youth specialist health response measures regarding adverse childhood events (ACE) and their impact on young people's health, including speciality programs for young parents
* This position is located at the clinical health service location of MYH West (which is also the home base for the MYH Young Parents. Program), where multiple support programs run, including the Talking Realities FLO Program and various grant funded Young Parents support Programs.
* MYH programs at this site, focus on building and strengthening safe and positive relationships for all young people accessing our health services but with special focus on young parents and their children.
* Building these protective factors improve children and young people's resilience and reduce the impacts of ACE's on their future health and disrupting intergenerational cycles.
