The successful applicant for this position will be located at Gloucester Community Health Service and be a part of the Generalist Care multi-disciplinary team. You will be carrying out home visits within the community providing Palliative Care Nursing to patients. In the immediate work environment, you will work among Community Health Nursing, a Nurse Led Clinic, Child and Family Services and many outreach services.
As a Palliative Care Registered Nurse, you will provide compassionate, person-centred care for individuals with life-limiting illnesses in the community. You will provide specialist symptom management, case coordination, and medication management, working collaboratively with the Lower Mid North Coast Specialist Palliative Care team to address patients' physical, emotional, and social needs.

The broader Generalist Care multi-disciplinary team includes Wound Management, Cardiac & Respiratory Rehabilitation, Continence, Dietetics, Diabetes Education, Wound Specialist, Immunisation and Audiometry.
The Palliative Care team consists of a Clinical Nurse Consultant, Nursing Staff, Bereavement Counsellor and Palliative Care Volunteers; each small team is based to cover the four areas across Lower Mid North Coast: Taree, Forster, Gloucester and Bulahdelah.

What We're Looking for in a Candidate
* Current Authority to Practice as a Registered Nurse with AHPRA
* You are a dedicated Registered Nurse with a passion for palliative care and a holistic, person-centred approach.
* You bring empathy, resilience, and outstanding communication skills, along with the ability to work effectively in a multidisciplinary team.
* You have strong organisational abilities, apply evidence-based practice and critical thinking and are confident navigating the complexities of community-based palliative nursing.
* You are self-motivated, caring, compassionate and have post graduate studies in Palliative Care or willing to work towards same.
