The Team
We are seeking an experienced health social worker to be part of our Community & Outpatient Social Work team, based at New Norfolk District Hospital, supporting consumers in the community, outpatient and hospital setting.
The Role
Within a primary health care framework, provide a social work service to individuals, families, groups and communities.
Deliver accessible and equitable social work services to adults in Southern Tasmania who experience psychosocial health issues.
Provide an effective, holistic psychosocial service that is evidence‐informed, locally appropriate and sustainable.
As a senior member of the health care team provide support to leadership in developing, implementing and monitoring policies and practices that reflect the priorities and standards of the service.
Promote community awareness work with other agencies, organisations and the wider community to develop an integrated approach to delivering health services.
Qualifications / Experience
* Extensive experience as a practising social worker, together with substantial knowledge and skills in the provision of a Social Work service in community settings and sound knowledge and understanding of the principles of primary health care and of current state and national health vision and policy.
* Knowledge of the principles of community development and skills in developing and evaluating health promotion programmes, understanding and experience in community recovery assessment and response plus knowledge of community resources and the ability to provide support to health professionals, non‐government service providers and the wider community in relation to health care needs.
* Proven high level communication skills, including the ability to work collaboratively as part of an interdisciplinary health care team and to build positive working relationships with other service providers, local support organisations and the wider community.
Salary and Benefits
Salary: $110,425 to $121,907 per annum pro rata. Our Employer 12% superannuation contribution is on top of this amount.

Eligibility
* Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers.
* Current Driver's Licence—driving required to access rural areas.
Details of Appointment
Fixed term part‐time, day worker position, working 57 hours per fortnight, commencing as soon as possible for a period of 12 months.
