Even if you only meet some of the criteria, we'd love to hear from you.About this ProgramThe Grampians Youth Prevention and Recovery Care (YPARC) program is a collaborative service between Wellways and Grampians Health Services.The service is a step-down and step-up sub-acute mental health service for those within the catchment areas of Grampians Health. YPARCs are in a community setting and provide an option for people who are becoming unwell, or who are in the early stages of recovery from an acute illness and need a short period of additional support to strengthen their gains from spending time in an inpatient setting. They are supported to consolidate their community transition and recovery treatment plans. Clinical intervention is provided to YPARC through the Area Mental Health Service. The program is staffed 24/7.We work in close partnership with clinical and other partners, the person experiencing mental health issues, their families and friends. It is our goal to assist people in their own pathway to recovery.About the opportunityThis role supports Wellways to deliver a personal and valued service to people and community in the Grampians region.The Key Deliverables of the role are:Develop individual service plans with each participant which incorporate individual goals focusing on skill and knowledge development while working towards recoveryEncourage linkages into a range of activities to support achievement of goals, including all other Wellways servicesEnsure that all operational and administrative requirements are met including regular reporting requirements and records maintenanceMaintain safe work practices and a safe and healthy environment in accordance with occupational health and safety policies and legislationEncourage linkages into a range of activities to support achievement of goals, including all other Wellways servicesActively participating in team meetings, service planning sessions, PDR supervision and staff development activitiesRepresenting Wellways in a variety of settings, including other agencies, members, participants, carers and families to raise awareness of mental health issues at the local level and to 'market' the organization regionallyOther areas of responsibility and accountability include:Contributing to a positive team culture and effective service delivery.Intake and assessment of YPARC participantsDelivery of group programming as per scheduleDaily keywork sessions with participantsSupporting the facility cleanliness and maintenance as neededYou will also need:Certificate IV or Diploma in Mental Health, AOD, or a related community services field, and relevant experience or an equivalent combination of relevant qualifications and experience.Current Working with Children Check and NDIS Workers Screening CheckCurrent valid Driver's License and the ability to undertake travel for the roleRight to Work within AustraliaSatisfactory National Police Records CheckNDIS Worker Orientation Module CertificateAbout UsWellways connects people, strengthens families, and transforms communities for individuals of all ages experiencing disabilities or challenges to their social and emotional wellbeing.
