Job Title: Support Worker - Make a Real Difference
About the Role:
We are seeking compassionate and dedicated individuals to join our team of support workers. As a support worker, you will play a vital role in providing high-quality care and support to people with disabilities.
You will work closely with clients to understand their individual needs and preferences, and develop personalized support plans to help them achieve their goals. This may involve assisting with daily living tasks, such as personal care, meal preparation, and household chores, as well as supporting clients with leisure activities and community participation.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, be able to work independently and as part of a team, and have a strong commitment to delivering person-centered care. You will also require relevant qualifications and/or experience working with people with disabilities.
Key Responsibilities:
* Provide high-quality support and care to people with disabilities
* Develop and implement individualized support plans to meet client needs and goals
* Assist clients with daily living tasks, such as personal care, meal preparation, and household chores
* Support clients with leisure activities and community participation
* Work collaboratively with other healthcare professionals to ensure comprehensive care and support
Required Skills and Qualifications:
