Case Co-ordinator – Evening Shifts
Are you a skilled Registered Nurse ready to take the next step in your career? Silverchain is seeking a passionate Case Coordinator to lead a clinical team in delivering integrated community care to clients with complex acute and aged care needs.
In this pivotal role, you will coordinate clinical and physical resources, provide direct client care, and serve as a key escalation point for clinical challenges. You will also collaborate closely with the Clinical Nurse Manager to achieve service delivery goals and uphold Silverchain’s commitment to excellence.

The role

This role is available on a part time basis of 45 hours per fortnight – the work/life balance you are looking for!
7.5-hour shifts consist of:
Week 1: Monday & Tuesday
Week 2: Saturday, Sunday, Monday and Tuesday
- Lead and support a dedicated clinical team, fostering growth, competence, and emotional wellbeing
- Coordinate resources to deliver person-centred, evidence-based care that meets client needs
- Deliver hands-on clinical care and resolve complex issues with confidence and compassion
- Act as a key escalation point for nurses managing client deterioration and other clinical challenges
- Drive communication through team meetings, stakeholder engagement, and documentation excellence
- Support onboarding, mentoring, and student placements to build a strong, future-ready workforce
What we’re looking for
You have passion with purpose and are driven by excellence. Caring is a profession and a calling, and your work and support enriches everyone. In addition, you will have the following skills:
- Bachelor of Nursing with current AHPRA registration
- Minimum five years post-graduate nursing experience with demonstrated clinical leadership and resource coordination
- Proven ability to meet KPIs and make innovative decisions to support quality client care
- Skilled in mentoring staff and supporting professional development
- Excellent communication, negotiation, and problem-solving skills
- Comfortable working autonomously and collaboratively in dynamic, changing environments
- A current Australian driver’s licence
