Remuneration: $37.77 p/hr + 12% Super + Not for Profit Salary Packaging (increases take home pay) + Allowances & Travel Reimbursement (KM's) + Employee discounts.
About the Role

As a Senior Personal Carer, you will conduct initial client intake and screening assessments, develop and review individualized care plans with a client‐centred approach, and provide quality care based on clients' needs.
Key Responsibilities
* Conduct thorough initial screening assessments and develop care plans for personal self‐care, general house cleaning, social support, respite, transport, and shopping assistance.
* Manage client information in CRM & CMS ensuring all details and case notes are kept up to date.
* Engage in ongoing care discussions, planning, assessment, reviews, and evaluate goals and outcomes in consultation with the care team.
* Spend time on the road completing scheduled visits, interacting one‐on‐one with clients in their homes to support daily activities.
* Provide care in accordance with the individualized Care Plan.
* Support community‐based recreational and social activities and provide transportation and assistance with shopping and appointments.
Role Requirements
* Respite Centre Experience.
* Highly Regarded Certificate III Aged Care, Individual Support or an equivalent qualification.
* Essential: Current Australian Driver's Licence (Green Provisional minimum); International licences will not be considered.
* Roadworthy & insured vehicle (Third Party Property minimum) to use for work.
* Essential Experience with technology incl. smartphones, tablets, laptops etc.
* Confidence to satisfy relevant probity checks required by legislation or policies.
* This role will be based on mainland (Alexandra Hills) with requirement to cover Macleay & Russell Islands.
