Overview

Clinical Nurse Consultant - Voluntary Assisted Dying Care Navigator Service

SMHS is seeking a Clinical Nurse Consultant to coordinate Voluntary Assisted Dying (VAD) services for SMHS inpatients and outpatients. The CNC will provide initial assessment and discussion with patients and families to ascertain their understanding of the VAD process, and when requested will facilitate access to VAD clinicians for assessment and coordinate the VAD process for patients. The CNC will liaise with treating teams to encourage completion of goals of patient care, referral to palliative care and discussions around advance care planning for both inpatients and outpatients across South Metropolitan Health Service (SMHS). Responsible for ensuring the safe delivery of VAD services within SMHS, the CNC delivers education and training for SMHS staff to ensure ongoing alignment with policies, standards and legislation regarding VAD, implementing changes to clinical practice; and initiating research and quality improvement activities as well as monitoring, identifying and reporting on risks associated with VAD and End of Life (EoL).

Responsibilities

* Coordinate Voluntary Assisted Dying (VAD) services for SMHS inpatients and outpatients.
* Assist patients, their families and SMHS staff in matters relating to VAD and End of Life Care (EoLC) choices.
* Provide initial assessment and discussion with patients and families to ascertain understanding of the VAD process; facilitate access to VAD clinicians for assessment; coordinate the VAD process for patients.
* Liaise with treating teams to encourage completion of goals of patient care, referral to palliative care and discussions around advance care planning for both inpatients and outpatients across SMHS.
* Deliver education and training for SMHS staff to ensure ongoing alignment with policies, standards and legislation regarding VAD; implement changes to clinical practice.
* Initiate research and quality improvement activities as well as monitor, identify and report on risks associated with VAD and EoL.
