About Us
Clarity Health Care is an accredited mental health service delivering a multidisciplinary, responsive program offering care to people with any form of mental health concern or mental illness. Our programs include psychiatrists, psychologists, occupational therapists, social workers and registered nurses. Evidence-based care is delivered through outreach services, clinic-based appointments, telehealth and group programs, all grounded in the recovery and bio‐psycho‐social models.
Your Key Responsibilities Include
* Engaging in up to 5 client consults per 7.5‐hour day, providing personalised care and support.
* Conducting intake assessments and developing tailored intervention plans in collaboration with clients and their families.
* Delivering ongoing evidence‐based interventions to consumers and their families as defined by the initial assessment.
* Advocating for clients and coordinating with other clinicians and stakeholders to enhance interventions and well‐being.
* Monitoring and managing clients' mental health and well‐being, collaborating with health professionals as needed.
* Participating in clinical meetings, training sessions, and professional development activities to enhance your expertise.
* Developing, reviewing, and closing treatment plans in partnership with consumers and their identified supports.
* Assessing, monitoring, and managing the client's mental health, wellbeing (including physical health concerns) and risks, and collaborating with other professionals, partner organisations and stakeholders as required.
* Administering and facilitating psychometric testing as clinically indicated, working within Clarity Health Care's framework of early intervention, assertive treatment, anti‐stigma, diversity, inclusion, and non‐discrimination.
* Identifying and taking an active role in quality and service improvement activities.
* Developing clinical practice knowledge and expertise through a commitment to professional development and active learning within the team.
* Writing reports as required for internal/external use, e.g., assessment formulation, incident reports, funding body reports, and court hearings.
What We're Looking For
* Current AASW (Social Worker) accreditation in mental health.
* Demonstrated experience in comprehensive mental health assessments and evidence‐based interventions.
* Strong interpersonal and communication skills, and the ability to build rapport and work collaboratively in a multi‐disciplinary team.
* Experience with multiple computer programs and the ability to produce well‐written reports.
* Passion for making a difference and the ability to thrive in a fast‐paced, solution‐focused environment.
* Effective time management and organisational skills, with the ability to meet program delivery KPIs.
