Case Manager – Community Support Specialist
The Connecting Pathways Program is a comprehensive healing initiative catering to Aboriginal and Torres Strait Islander individuals affected by family violence. This role entails delivering holistic health and wellbeing services in various programs at Wurli, ensuring seamless linkage between the organization, clients, families, and communities.
Key Responsibilities:
* Foster high standards of service delivery by adhering to organisational and program policies.
* Promote case management services and support to clients as referred into the program.
* Maintain legislative compliance in service delivery, including mandatory reporting requirements.
* Contribute to intervention actions in community and family situations prone to violence associated with anti-social behavior.
Requirements:
* Certificate IV in Community Services or equivalent experience.
* Demonstrate exceptional community and client relationship-building skills, accompanied by sensitive and effective communication with Indigenous people.
* Possess current National Police Records Check and Working with Children's Clearance.
* Hold a valid Northern Territory Driver's License.
