1. Queen Elizabeth Hospital, Rehabilitation Services – Day Rehabilitation Service (DRS) / Rehabilitation in the Home (RITH)
2. AHP2 $94,997 to $110,052 p.a. (pro rata) + super + salary packaging
3. Ongoing Full Time and Part Time positions available

About the Role

If you are a social worker passionate about making a meaningful difference in the lives of people throughout their rehabilitation and recovery journey, we would love for you to join our team! The Social workers across DRS and RITH are a key part of the multi-disciplinary rehabilitation team and focus on achieving goals that optimise outcomes for individuals and their families. The role has the autonomy to tailor social work intervention plans to ensure a holistic and client-centred approach to recovery.

The successful applicant will be a motivated and compassionate social worker who thrives within a multidisciplinary rehabilitation team. You will be supported by experienced Senior Social Workers, connected with enthusiastic and like-minded peers, and encouraged to pursue ongoing professional development opportunities. This role provides the opportunity to:

4. Deliver counselling and psychosocial support
5. Support discharge planning and facilitate community referrals
6. Collaborate closely with multidisciplinary healthcare professionals
7. Support and guide students throughout their learning journey
8. Contribute to service development, quality improvement, and person-centred care planning

About You

You are a compassionate and skilled Social Worker who delivers person centred care and supports positive outcomes for patients and families.

9. Support diverse clients with empathy and professionalism
10. Social Work degree with AASW membership eligibility
11. Deliver assessment, counselling and therapeutic support
12. Work collaboratively across multidisciplinary teams
