NURSE CO-ORDINATOR – DISABILITY SUPPORT

The Role
We are seeking a dedicated Nurse Co-ordinator – Disability Support (SCHADS Level 6) to provide nursing guidance, support staff, and assist with care planning across our disability and community services. This role involves hands-on nursing support, coordinating participant care, and ensuring our services meet high standards of safety and quality.
Key Responsibilities
· Participant Care & Health Support:
o Provide nursing support in the administration of medications and management of complex health needs, including wound and stoma care.
o Conduct health assessments and monitor participant wellbeing.
· Care Planning & Coordination:
o Develop and implement individualised care plans tailored to participants' needs.
o Collaborate with families, allied health professionals, and support coordinators to ensure seamless care delivery.
· Staff Support & Mentoring:
o Support and mentor support workers and other nurses in delivering participant care.
o Conduct skills assessments and provide training to ensure safe and competent care practices.
· Compliance & Quality:
o Ensure care practices meet NDIS and organisational standards.
o Maintain accurate documentation and assist in implementing policies and procedures.
· Communication & Liaison:
o Coordinate appointments, support participant interactions, and provide updates on health and care needs.
Who You Will Support
· Individuals with a disability requiring ongoing or complex health support
· Staff delivering disability and community care services
What We're Looking For
· Registered Nurse (RN) with 3+ years' experience in hospital or primary care.
· Postgraduate qualifications desirable.
· Empathetic, caring, competent, and confident.
· Strong understanding of RN responsibilities and clinical governance.
Minimum Essential Criteria
· Working With Children Check (for paid employment)
· NDIS Worker Check Clearance
· Senior First Aid & CPR
· National Police Certificate (issued within the last 3 months)
· NSW Driver's Licence
