Case Manager

* We provide case management services to ensure client wellbeing and positive outcomes, working collaboratively with carers and stakeholders in Tamworth or Armidale.
* We coordinate case plans, monitor progress, and maintain compliance with relevant standards and legislation.
This is a maximum-term part-time position working 6-hour shifts 3 days a week. With integrity and resilience, you will:
* Deliver case management services including assessment, planning and intervention to support client safety and wellbeing.
* You will facilitate client visits, monitor quality interactions, and produce professional reports.
* You will recruit, assess, review and train carers to maintain required standards aligned with client case plans.
* You will maintain accurate and up-to-date client records.
* You will engage stakeholders through positive relationships and clear communication of roles and responsibilities.
* You will monitor risks and progress towards agreed case plan goals with achievable outcomes.
* You will contribute to continuous improvement by assisting with policies, procedures and work practices.
* You will ensure compliance with funding body requirements, organisational policies, and evidence-based strategies.
* You will promote cultural awareness and respect, including consideration for Aboriginal and Torres Strait Islander peoples.
* You will participate in professional development, training, and safety initiatives.
