About Sunraysia Community Health Services

Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The organisation is a source of primary and preventive care for our community members who are at risk of poor health outcomes, and experience acute health disparities. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.

The Youth Services Team delivers a range of individual and small-group services onsite at HomeBase and within the community, including in partnership with external agencies. A key focus of the program is genuine engagement and collaboration with young people to ensure services are responsive to community needs and support positive outcomes.

The program includes the Youth Crime Prevention Program (YCPP) streams of Mentoring and Coaching Supports and Pro-social Activity and Engagement. These streams provide comprehensive, strengths-based support to young people who are at risk of involvement in the justice system, with a focus on early intervention, increasing protective factors, and reducing barriers to positive participation in community life.

Through mentoring, coaching, and structured pro-social activities, the program supports young people to strengthen social connection, build resilience and life skills, improve engagement in education, training and employment, and develop positive relationships with peers, family, and community.

The role is responsible for delivering flexible, youth-centred support to young people participating in the Youth Crime Prevention Program (YCPP). This includes providing mentoring and coaching support, facilitating pro-social activities and community engagement opportunities, and working collaboratively with young people to identify goals and pathways for positive development.

A strong focus of the role is to promote empowerment, inclusion, wellbeing, and community connection while supporting young people to reduce risk factors associated with involvement in the justice system and strengthen long-term protective factors.

Key Selection Criteria
* Relevant qualifications and/or demonstrated equivalent experience working with young people aged 10–17 years from diverse cultural, social, and community backgrounds.
* Experience planning, facilitating, and delivering youth programs, group activities, and community-based initiatives.
* Current Victorian Driver Licence.
* Highly developed interpersonal, communication, and engagement skills, with the ability to build positive and respectful relationships with young people.
* Demonstrated understanding of trauma-informed practice and relationship-based approaches to working with young people and families.
* Sound understanding of the social determinants of health and the impact these can have on young people's wellbeing, development, and engagement.
* Understanding of privacy, confidentiality, and professional boundaries when working with young people and their families.
* Current Victorian Working with Children Check (Employee), or the ability to obtain and maintain one prior to commencement.
