We are looking for an experienced and enthusiastic Dietitian to join our Perth Metro team servicing the Mandurah and Rockingham areas, on Permanent Part-time basis.
The Dietitian provides clinical assessment and dietetic education within our diverse programs and services which include the Commonwealth Home Support Program, NDIS Therapy, Healthy Lifestyle Services, and fee-for-service, giving you opportunities to work with a wide range of people in a variety of service settings including 360 Health clinics and home visits.
Key Responsibilities
The Dietitian carries out clinical assessment and delivers dietetic education tailored to clients across various programs such as the Commonwealth Home Support Program, NDIS Therapy, Healthy Lifestyle Services, and fee-for-service. The role involves case management of clients with chronic conditions or who are ageing, developing high quality care plans, case notes and written reports, and creating a comfortable environment during home visits.

Qualifications
* Relevant tertiary qualification and Dietitians Australia membership.
* At least 1 year of relevant experience in a similar role.
* Good knowledge of and ability to provide education on the impacts of ageing as well as chronic disease management and prevention.
* Experience in case management of clients with chronic conditions or, who are ageing.
* Ability to develop high quality care plans, case notes and written reports.
* Diligent, can‐do attitude and ability to thrive in a dynamic environment.
* Excellent manner and ability to create a comfortable environment for clients during home visits.
* National police certificate dated within 6 months, plus a working with children check and NDIS worker check for maximum program flexibility.
