Headspace is Australia's National Youth Mental Health Foundation, providing services across Australia, in metropolitan, regional, rural and remote areas, supporting young people and family to be mentally healthy and engaged in their communities.
This role sits within the Enhanced headspace Service (EhS) utilising a unique dual-worker model, working alongside the Mental Health Clinician to support young people facing moderate-to-severe mental health challenges and delivering quality care to every young person you work with.
EhS combines expert psychological support with practical, real-world guidance focusing on helping young people build skills, strengthen wellbeing, and stay connected. With flexible, personalised care, including outreach and in-centre support, EhS meets young people where they are, empowering them to achieve meaningful, lasting outcomes.
As a Youth & Family Care Coordinator, you'll work with the Clinical Lead to deliver psychosocial, non-clinical interventions to young people, including social connection and skill development. You'll conduct risk assessments, coordinate implantation of care plans and work closely with internal and external stakeholders within the community to deliver the best support for the clients.
About You
* Experience providing care coordination and psychosocial support in a mental health setting, preferably with young people and families.
* Experience working with young peoples' mental health and well-being, leading to the provision of youth friendly, inclusive and high-quality services, with a recovery and strengths-based focus.
* Experience in developing, managing and implementing care plans for clients with mental health difficulties.
* Experience conducting risk assessments of young people and developing and implementing risk management plans, including crisis management.
* Experience in working with hard-to-reach groups such as Aboriginal and Torres Strait Island young people, Culturally and Linguistically Diverse and LGBTQIA+.

Requirements
* Tertiary qualifications and/or relevant experience as a Youth Worker, Care Coordinator, Support Worker or other relevant field in health, social or community services
* Current Driver Licence
* Current Blue Card
* Current National Police Check
