In collaboration with the multidisciplinary team, this role will ensure that treatment and care is planned and delivered in partnership with patients and families.The CNC also provides expert clinical advice to patients, carers and other health care professionals and facilitates care management plans for patients who are referred to GenesisCare and who are discharged back to their health service provider.
