- Disability Services Coordinator/ Residential Facilities Coordinator**

**Work type**:Full-time

**Purpose of this job**: The purpose of this role is to provide Senami's clients with support to achieve their goals set out in their NDIS plans. The disability services coordinator is responsible for providing guidance and coordinating the day to day operations of the residential facilities settings to ensure best service delivery for our clients. This position will work closely with our Registered Nurse, Team Leaders, support workers and clients living with disability and mental health diagnosis.

The role of a disability services coordinator is to evaluate support programs, coordinate training and conduct mental health assessments for clients living with mental health, disability and other comorbidities. This position will require you to provide holistic and individualized person care plans, empowerment-based interventions and engage in services to promote development and well-being of our clients. The role will entail service delivery from residential and community-based settings across Adelaide.

**Key relationships/interactions**:Registered Nurse, Team leaders, mental health support workers, office administration team, existing and potential clients as well as their relevant stakeholders.

**Qualifications, experience and licenses**:

- Tertiary qualification in Allied Health and/or Health Sciences
- Nursing or Allied Health equivalent (Mental Health, Social Work and/or Psychological studies background also encouraged to apply)
- Minimum 3 to 5 years' experience within the mental health and disability field in coordinating responsive individualized support to individuals living with disabilities
- AHPRA registration (desired)
- Current and valid driver's license

**Mandatory knowledge and skills**:

- First-aid, Mental health first aid and MAPA (Safety Intervention Training) accreditation
- Access to own reliable vehicle and willing to travel when necessary
- Valid working with children, NDIS and DCSI checks
- Valid National Police Clearance
- Valid working rights in Australia
- High level of written and verbal communication with excellent interpersonal skills
- Computer literacy with common Microsoft platforms and patient management systems (Careview etc.)
- Excellent triage skills
- Supportive, reliable, highly motivated, energetic personality
- Excellent leadership, administration and organizational skills
- Proficiency in recording patient care details
- Organizational skills with the ability to work autonomously within the multi-disciplinary team
- Ability to effectively plan and prioritize tasks
- Ability to create and adhere to structured routines
- Ability to maintain professional and accurate client records
- Excellent decision-making skills
- Ability to establish and maintain links and positive professional relationships with client families

**Desired knowledge and skills**:

- Sound knowledge and understanding of SA and national legislation governing area of practice (Mental Health Act)
- Experience in:

- the disability sector and knowledge in regards to NDIS legislations, requirements and standards
- supporting individuals with challenging behaviors
- supervising and managing a diverse group of staff members
- delivering group education and training
- planning and evaluating group-based activities for students, staff and/or the wider community
- Clinical knowledge and skills
- Documentation and report concerns

**Key accountabilities**:

- Acting in the best interest of the company and our clients, including confidentiality, privacy, individual choice and decision making
- Follow the instructions gave by Supervisor and direct to related teams or stakeholders
- Deliver and review high-quality care to residential clients under the supervision of Registered Nurse and Company Manager.
- Conduct on-going house meeting, mental health and risk assessments, welfare checks, training sessions for staffs
- Document and Report all care concern from the house or staffs directly to Supervisor.
- Providing physical and emotional support to clients and staffs if needed and continue monitoring the process.
- Maintaining all client's health, care plans, community programs, related documentation, policies and procedures are always up to date
- Ensure all standards of care are complies with the ethics and clinical
