Key responsibilities include:
Risk Assessments:
The case manager will conduct thorough risk assessments to identify potential dangers to the client and their family.
Collaboration with Services:
The case manager will work closely with various services, including healthcare professionals, social workers and emergency services, to ensure the client receives comprehensive support.
Safety and Independence Goals:
The case manager will support clients to achieve their safety and independence goals, which may include developing a safety plan, accessing counseling services or finding employment opportunities.
This is a full-time position and the successful candidate will be required to work in Port Hedland, Western Australia.
