Position Overview
SWSLHD is seeking an experienced physiotherapist to join the community-based Liverpool Transitional Aged Care Program (TACP) as a full-time permanent level 3 physiotherapist. The role is based within a multidisciplinary team at the Liverpool TACP site and focuses on the reablement of older adults living in the community.
Responsibilities
* Assess TACP clients in their homes, setting holistic goals, developing client-centred treatment plans and discharge planning.
* Commit to evidence-based practice and lead quality improvement initiatives within the physiotherapy team.
* Provide ongoing education, training and supervision for level 1/2 physiotherapists, allied health assistants and students.
* Participate in guideline and policy review, staff education, and district-wide quality improvement/EBP projects.
Qualifications
* Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
* Experience working in aged care and/or rehabilitation.
* Ability to work independently and collaboratively within a multidisciplinary team.
* Solid communication, problem-solving and leadership skills.
