The Role
As a Service Coordinator, you'll plan, coordinate, and monitor services under the Support at Home 2025 program & NDIS. This role ensures clients receive timely, quality, and individualised support that aligns with their goals and wellbeing.
Key Responsibilities
* Conduct client assessments and develop personalised Care Plans
* Coordinate and monitor services within approved budgets
* Liaise with clients, carers, and allied health professionals
* Maintain accurate client records and documentationSupport and guide community care staff
* Support at Home quality standards & NDIS standards
About You
You're an organised, empathetic professional who enjoys building relationships and delivering quality client outcomes.
Essential
* Certificate IV in Ageing, Disability or Community Services
* Current driver's licence and reliable vehicle
* Current First Aid & CPR Certificate
* NDISWSC
* WWCC
* Experience in service coordination or case management
* Excellent communication and computer skills
Desirable
* Experience with My Aged Care and aged care compliance systems
* Experience with NDIS and compliance systems
