Job Title: Transition Coach
The Mission Australia Home Stretch WA program is a groundbreaking initiative co-designed with young people, Aboriginal community members, and the broader support network. Our goal is to transform the transition from Out-of-Home Care to independence.
As a Transition Coach Case Manager based on site in Bunbury, you will play an integral role in this innovative service. Your primary objective will be to provide individualized, professional support and case coordination to young people extending crucial resources and guidance as they navigate the complex journey to independence.

* Develop a young person's natural networks and assist them to build and maintain connection to culture, country, family, and community supports.
* Offer and broker access to a safety net of financial support and resources through the Home Stretch WA service offer.
* Provide tailored support services using a culturally safe and strengths-based approach and case coordination that is consistent with the Home Stretch WA Transition Coach Practice Principles.
* Work collaboratively with relevant stakeholders including the Department of Communities Child Protection staff, foster families, carers, the young person's kin, community, and people identified as significant in their lives.
* Ensure the right to self-determination drives your practice, while promoting the wellbeing of young people.
To be successful in this role, you will require:
* A Degree or Diploma in Social Welfare/Social Science or similar, and a minimum of two years' experience in a similar role.
* Comprehensive understanding of the barriers and issues affecting young people who have been in state care, including those disengaged from education, training, and employment.
* Ability to work in a culturally safe, trauma-informed manner, with strong knowledge of Aboriginal culture.
* Proven experience in providing case management, assessment, counselling, and crisis intervention for vulnerable young people with complex needs.
* Strong interpersonal and networking skills, with the ability to build effective relationships with Aboriginal communities and services.
* Excellent written and verbal communication skills, along with high-level computer literacy.
* Ability to work flexibly in a solutions-focused manner, both independently and as part of a diverse team.
