Camcare was originally established by community and continues to provide a welcoming service in Boroondara. Camcare, a CISVic member organisation, is supported by around 70 volunteers to deliver community information and support, emergency relief and material aid, and family services.

Camcare is part of Access Health and Community (AccessHC), a leading not‐for‐profit provider of health and community services with over 150 years of experience delivering inclusive, person‐centred care across Melbourne's inner and outer east and northeast. The Opportunity

Every day, people across Boroondara turn to Camcare for compassionate, practical, and immediate support. As financial pressures rise and pockets of disadvantage deepen, our Community Support service is seeing more individuals and families seeking guidance, material aid, and pathways to health and wellbeing services.

To strengthen our response, we are introducing a Complex Care Coordinator—a newly created role within our Community Support team, funded through the City of Boroondara Community Services Triennial Grant 2026‐29. This is a rare opportunity to shape a role from the ground up and make a meaningful impact in the lives of community members experiencing complex challenges.

This role is ideal for someone who thrives in a community‐based environment, values person‐centred practice, and is motivated by the opportunity to influence service design and outcomes.

About The Role

The Complex Care Coordinator will play a pivotal role in enhancing the support provided to clients presenting with multiple or layered needs. Working alongside our dedicated volunteer Community Support Workers, you will offer professional secondary consultation, short‐term case coordination, and targeted interventions that address the social determinants of health.

This is a Max term Part time 0.6 EFT position funded to June 2029. Reporting to the Manager, Community Support, you will work across the Camcare sites of Camberwell, Ashburton and North Balwyn.

Key Responsibilities
* Provide care coordination to enable people seeking complex support from Camcare to access the supports and services available to assist them.
* Support internal and external collaboration to support referrals and service navigation.
* Work with trained volunteer Community Support Workers to create a service model that supports clients presenting with complex issues.
* Use your skills and experience to take a client‐centred and culturally responsive approach to provide a biopsychosocial assessment of immediate and longer‐term needs and co‐develop achievable goals.
Key Selection Criteria
* Tertiary Qualifications in Social Work or related discipline.
* Minimum of 5 years' experience in Social Work (or related role) within a community services environment.
* Demonstrated experience working with a team/volunteers.
* Demonstrated experience in care coordination or case work with diverse communities and those hardly reached by the service sector to alleviate immediate biopsychosocial needs and empower clients to achieve their longer‐term goals.
* Demonstrated experience navigating and referring into the health and community sectors.
* Desirable: proficiency in a community language other than English (e.g. Mandarin).
Compliance Requirements
* National Police Check, Working with Children Check.
* Evidence of the right to work in Australia and a valid Driver's Licence.
