Senior Case Manager (Reconnect)

We're looking for an experienced Senior Case Manager (Reconnect) to join our Community Services team, supporting young people and their families through complex, real-life challenges.
Role Type: Part Time Maximum Term role until 30 June 2027 – 45 hours fortnight.
Remuneration: $46.77 to $50.33 per hour + Super + Salary Packaging + Employee discounts.
Responsibilities
* Deliver end-to-end case management for young people and families.
* Conduct risk and needs assessments and develop tailored support plans.
* Provide counselling, mediation, advocacy, and family support.
* Support home stability, life skills development, and transitions.
* Work collaboratively with Child Safety, Youth Justice, Health, Education, and other stakeholders.
* Facilitate individual and group programs.
* Maintain accurate case notes, reporting, and data collection.
* Participate in community networks and service development initiatives.
Qualifications
* Degree in Human Services, Social Work, Psychology, or related field.
* Strong experience in case management within youth, family, or community services.
* Ability to manage complex cases using therapeutic and evidence-based approaches.
* Confidence working with multiple stakeholders and high-risk situations.
* Excellent communication, documentation, and organisational skills.
* Current Blue Card, NDIS Worker Screening, and QLD Driver's Licence.
