Overview
Western Sydney Local Health District is recruiting a Social Worker (Level 3) for a temporary part‐time position (up to 12months) delivering community‐based social work care. The role will involve advanced clinical leadership for complex community health clients and families across multiple sites in Mount Druitt, The Hills, Parramatta, Auburn & Blacktown Community Health Centres. This part‐time role requires 16hours per week, with job share considered. Salary: $58.95‐$60.86 per hour (including superannuation and salary packaging). Annual salary ranges $116,892–$120,680.
Work Environment
This role sits within Community Health services and is delivered at several community health centres across greater Sydney. The key characteristics are:
* Community‐based social work services across multiple sites.
* Reports to Program Lead and Service Manager.
* Works within multidisciplinary teams including medical and nursing staff.
* Supports service planning, evaluation, and quality initiatives.
* Collaborates with clients, carers, and external providers.
Key Responsibilities
* Provide senior‐level clinical social work services in complex and sensitive care contexts.
* Deliver advanced assessment and therapeutic interventions, including palliative care.
* Work autonomously with minimal supervision in complex clinical scenarios.
* Support multidisciplinary care planning and coordination.
* Lead service improvement, education and quality initiatives.
* Provide supervision and mentoring to staff and students.
Essential Criteria
* Degree in Social Work with eligibility for membership of the Australian Association of Social Work.
Selection Criteria
* Degree in Social Work with eligibility for membership of the Australian Association of Social Work, demonstrating advanced clinical knowledge, skills and experience in community social work, including palliative care.
* Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
* Experience or capability in developing and implementing policies and procedures for service improvement, and in providing training and education with experience in staff and/or student supervision.
* Well‐developed time management skills with excellent written and verbal communication, including report writing skills.
* Demonstrated experience in quality assurance and/or clinical practice improvement activities.
* Demonstrated ability to work with consumers, families and carers during times of high family stress.
* Good computer skills, including experience using Cerner eMR or equivalent.
* Drivers license valid in Australia and willingness to use for work purposes.
