Dementia Support Australia is looking for passionate health and aged care professionals to join our growing team. We seek individuals who support and coach staff, families, and carers; have experience working with people living with dementia; understand clinical, social, and environmental factors that influence behaviour; solve problems creatively; communicate clearly and compassionately; and enjoy working hands‐on in diverse care settings.
The Role
Make a meaningful impact every day by supporting people living with dementia as they transition from hospital to home or into residential care. As a Dementia Support Coach, you'll provide practical, relationship‐based support, modelling and coaching for clients, carers and care staff across a variety of settings.
Fixed‐term contract until March 2028, based out of Coff Harbour or Lismore in NSW.

Responsibilities
* Provide on‐the‐ground support and implement relationship‐based strategies for clients, carers and care staff.
* Model and coach best‐practice approaches to help carers and aged care staff understand and support people living with dementia.
* Implement recommendation plans with guidance from the Team Leader, Dementia Consultant and Senior Consultant.
* Facilitate and support the trial and implementation of brokerage items for clients and carers.
* Collaborate with peers to build capability across aged care teams and support quality, person‐centred care.
Qualifications and Experience
* Strong organisational and time‐management skills in a busy work environment.
* Ability to work independently and collaboratively.
* A commitment to excellent customer service and constructive conflict resolution.
* A current driver's licence and safe driving record.
* Willingness to travel intrastate and interstate for up to five days at a time.
* Flexibility to work varying hours based on client and carer needs.
* Qualifications in a relevant field: Allied Health Assistant, Diversional Therapy, EN, Aged Care, Cert III/IV in Individual Support or Leisure and Health (highly desirable).
