We are a values-led organisation focused on Understanding, Respect, Wellbeing and Excellence. Across all services, our focus is on providing safe, consistent and person-centered support that is responsive to each participant's needs, goals, risks, preferences and support environment.

The Opportunity

We are seeking a Team Leader – Disability Support Services to lead our growing disability support team.

This role will initially focus on Community Access supports, while also supporting the team's continued development into 104A high-intensity supports. Over time, there is also potential for growth into Individual Living Options (ILO) and Supported Independent Living (SIL) style supports as the service area continues to develop.

This is a hands-on leadership role suited to someone who understands the importance of participant experience, workforce stability, proactive rostering, clear communication, quality documentation, compliance and service performance.

About the Role

As Team Leader, you will provide day-to-day leadership, supervision and mentoring to Support Workers and Senior Support Workers delivering individualized disability supports.

You will help ensure participants receive consistent, safe and high-quality supports, while also supporting workers with clear expectations, strong rostering practices, supervision, onboarding, training and performance development.

This role requires someone who can support end-to-end service coordination, including participant engagement, staff communication, roster oversight, workforce planning, caseload maintenance, service quality, documentation, compliance and operational follow-through.

You may also carry a small participant-facing caseload to model best practice, maintain service connection and support quality outcomes.

Key Responsibilities

* Lead and supervise Support Workers and Senior Support Workers delivering community access and individualized disability supports.
* Support participant-centered rostering, worker matching and continuity of support.
* Ensure rosters are practical, reliable and responsive to participant needs and worker availability.
* Maintain proactive and consistent communication with participants, families, workers and internal teams.
* Support recruitment, onboarding, induction, supervision and ongoing staff development.
* Apply strong HR-informed leadership practices, including performance feedback, accountability, documentation and staff support.
* Drive team performance, caseload maintenance, service utilization and KPI achievement.
* Monitor service activity and ensure the team is working toward agreed operational expectations and quality outcomes.
* Maintain high standards across documentation, including progress notes, risk assessments, support plans, incident reporting, service reviews, staff records and compliance requirements.
* Support quality assurance, safeguarding, incident follow-up and continuous improvement.
* Support the team's development into 104A high-intensity supports under appropriate training, delegation, clinical governance and oversight.
* Contribute to future service growth opportunities, including ILO and SIL-style supports where appropriate.
* Work collaboratively with internal clinical, compliance, intake, rostering, HR and leadership teams.
* Champion Activated Concepts' values of Understanding, Respect, Wellbeing and Excellence.

About You
