We are seeking a skilled Locum Clinical Pharmacist to join our team in providing clinical support and advice to patients across the UK.
As a successful candidate, you will be responsible for delivering high-quality patient care, working closely with healthcare professionals to ensure seamless service delivery and contributing to the improvement of access to healthcare.
Main Responsibilities
* Polypharmacy medication reviews and Service Medication Records (SMRs)
* Ensuring better access to healthcare services for patients
* Supporting practice staff with prescription and medication queries
* Medicine reconciliation and review post-discharge
* Conducting medical-related audits and working on service improvements with the practice
* Providing expert advice on medications to other clinicians and patients
* Delivering clinical leadership on medicines optimisation and quality improvement initiatives
* Educating patients on medication and disease prevention strategies
Requirements
Qualifications
* A relevant degree/qualification demonstrating registration as a clinical pharmacist with the General Pharmaceutical Council (GPHC)
* Completion of the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care pathway or exemption route if applicable
Experience
* A minimum of 12 months' experience working within general practice/primary care settings
* Proficiency in Electronic Medical Record Systems (EMIS or System 1) or both
