Job Overview
We are seeking a dedicated Social Worker to join the transitional aged care program at Fairfield Community Health Centre. This temporary part‐time role (24 hrs/week until May 2027) pays $38.51‐$54.94 per hour, inclusive of 12 % superannuation.
Responsibilities
* Provide holistic care to older clients in the community and a residential aged care facility.
* Collaborate with multidisciplinary team members to develop and implement client care plans.
* Support clients in achieving personal goals and maintain quality of life in their homes.
* Engage families and community services to enhance care delivery.
* Participate in professional development, clinical supervision, teaching and quality improvement activities.
Qualifications
* Social Work degree providing eligibility for Australian Association of Social Workers membership.
* Sound knowledge of the current My Aged Care guidelines and RACF respite/permanent placement processes.
* Australian C‐class driver's licence (P1 or P2 included). If licence is held from a state other than NSW, must obtain NSW C‐class licence within 3 months of appointment.
