Support Coordinator Role
As a Support Coordinator, you will play a pivotal role in empowering individuals with disability or age-related needs. This position involves working closely with clients and their families to coordinate access to essential services and resources.
Key Responsibilities:
* Assess client needs and develop tailored support plans to address their specific requirements.
* Liaise with service providers, healthcare professionals, government agencies, and community organisations to coordinate the delivery of essential services.
* Advocate on behalf of clients to ensure they receive the appropriate level of care and support.
* Regularly review and update client support plans to adapt to changing needs.
* Maintain detailed records and report on client progress and outcomes.
* Provide guidance and emotional support to clients and their families.
Experience within the Community sector and NDIS is recommended. The ideal candidate will possess:
* Extensive experience in a similar client-facing role within the aged care or disability support sector.
* Strong communication and interpersonal skills, with the ability to build trust and rapport with clients from diverse backgrounds.
* Excellent problem-solving and decision-making abilities, with a focus on delivering positive outcomes for clients.
* Proficient in the use of relevant software and technology to effectively manage client records and coordinate service delivery.
