CHQatHome Currently Supports The Following Programs
The CHQatHome service has multiple programs aimed at supporting children to receive appropriate care closer to home.
The service functions as a multidisciplinary team, including nursing, medical, physiotherapy, social work, pharmacy, psychology, dietetics, occupational therapy, speech pathology, psychology, administration and operational officers.
* Hospital in the (HITH)
* Post‐Acute Care (PAC)
* Hospital Avoidance (HA)
* Home Ventilation Program (HVP)
* Tracheostomy in Home Care (TIHC)
* Support of Palliative Care Service
About The Role
The CHQ at Home Support Worker is responsible for the support of a child or young person being cared for within their home environment.
The support worker will be trained and supported to provide safe care to a child or young person with high medical needs within their home or community environment under the supervision of the primary caregiver.
