About the role
Working within the Care team, you'll collaborate directly with participants, their families and other stakeholders to deliver high-quality assessments, personalised care planning and comprehensive case management which promotes participants goals and wellness outcomes.
Key responsibilities:
* Conduct HCP assessments and reviews in participants' homes
* Convert client needs into funding opportunities to maximise package utilisation
* Build strong relationships with participants, support networks and internal teams
* Deliver high-quality, person-centred care
* Provide comprehensive case management and advocacy
* Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports
* Manage incidents, feedback and complaints in line with ACQS requirements
* Record and maintain high quality, comprehensive documentation and case notes

* Travel is required with km reimbursement provided for use of your own vehicle
About you
* Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability), with ideally 3+ years industry experience
* Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways
* Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions
* A natural influencer who engages stakeholders and drives alignment and action
* Highly organised, agile and adept at managing competing priorities
* Committed to continuous improvement and service excellence
* Balances care outcomes with financial sustainability
