An opportunity exists to join the clinical team at Seymour: Permanent Part time – 0.8 FTE per fortnight.
The suitable applicant with need to demonstrate strong clinical assessment skills, ability to work collaboratively with external stakeholders community partners.
A Community clinician provides a critical role in the active coordination of care for people requiring further follow up linkages within the community.
The clinician will be required to communicate with referrers treating teams as required ensure that service continuity is maintained for consumers, supports family receiving assistance within mental health services.
