Key Responsibilities:
* Conduct thorough assessments of older adults to identify their needs and develop personalized care plans.
* Facilitate referrals to essential services that cater to the unique requirements of each individual.
* Promote retention and maximization of function and capability in daily living through proactive interventions.
* Collaborate closely with families and carers to recognize them as key stakeholders in the care process.
* Ensure all assessments are conducted in compliance with relevant Department of Health contractual requirements.
Requirements:
* At least 1 year of experience in aged care, including client assessments and/or service delivery.
* Tertiary education in a health-related discipline, such as nursing or social work.
* Current unrestricted registration with AHPRA or a relevant professional association.
* Proficiency in using Word, Excel, Outlook, and client management systems.
* Valid Australian driver's licence and comprehensive vehicle insurance for travel within designated areas.
