Care Partner Home Care Provider – Mandurah / Rockingham

This is a full-time role Mon - Fri

Our growth has been driven almost entirely by word‐of‐mouth referrals, and as a result, we are expanding our care partner team and seeking an additional experienced client‐focused care partner to join the Poppy's team.

About the Role

As a Care Partner, you will play a pivotal role in supporting older adults to maintain independence, wellbeing, and choice while living at home. Working within a strong and supportive structure, you will partner with clients and families, coordinate supports, and ensure care remains responsive, safe, and personalised.

Responsibilities
* Building trusted, lasting relationships with those you support
* Communicating clearly and respectfully with clients, families, and colleagues to support safe, high‐quality care
* Developing and reviewing person‐centred care plans
* Supporting clients to maintain independence, confidence, and quality of life
* Monitoring changes in circumstance and responding proactively
* Collaborating with families, associated providers, and allied health professionals
* Coordinating the care that the client needs and deserves, through referrals, organising supports and ordering essential products

What We're Looking For
* A great team player with a will to do the very best for our clients and care team
* Experience in community, aged care, or home care would be advantageous
* Sound judgement with the ability to understand individual needs and respond thoughtfully and very quickly
* Comfortable working independently while staying connected and supported within a collaborative team
* Strong computer and administration skills, including confidence using Microsoft Office, typing efficiently, managing multiple screens, and navigating a laptop/workstation with speed and accuracy in a fast‐paced environment
* Ability to complete clear, accurate, and timely case notes and documentation, with strong attention to detail and confidence working across multiple systems
* A natural communicator who builds trusted relationships with clients, families, and colleagues
* Warm, proactive, and compassionate, with a genuine commitment to supporting older adults
* Confident using technology, including care management systems and mobile documentation, to support effective and efficient care
