Home Care Registered Nurse – Clinical Partner

Permanent, part‐time position on the Far North Coast, 3 days a week.

About the role

The Clinical Partner works within HammondCare's Mission in Action to deliver high‐quality, person‐centred clinical care that supports clients to live well in their own home. The role provides evidence‐based, best‐practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed.

Responsibilities
* Assess clinical care needs and develop and review care plans.
* Deliver excellent clinical care in partnership with clients, their families and key stakeholders.
* Collaborate with the care management team to ensure the best outcomes for clients in the community setting.
* Provide guidance to care staff to ensure safe and continuous delivery of high‐quality clinical care.
* Advocate for clients' goals and support advocacy initiatives.
* Maintain accurate, digital health records and care coordination tools.
* Identify and implement innovations in care delivery and service improvement.
Qualifications
* Nursing qualification and current registration with AHPRA as a Registered Nurse (Division1, General).
* Current driver's licence and the ability to travel between HammondCare sites.
* Passion for and previous experience in client‐focused service delivery in aged and community/home care settings.
* Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
* Excellent communication and collaboration skills, with a commitment to advocacy and person‐centred care.
* Ability to work effectively in a multidisciplinary team and independently across multiple sites.
* Excellent administrative and computer skills, including digital health records and care coordination tools.
* Empathy, patience and a passion for delivering high‐quality, person‐centred care.
* Experience in aged care and community care, and ability to identify and implement innovations in care delivery.
