Telehealth Coordinator Role
Access Aged Care is seeking a skilled RN/EN to work in various aged care facilities across Perth. As a Telehealth Coordinator, you will be responsible for coordinating and facilitating Telehealth consults with GPs, Geriatricians, and Physicians.
Key Responsibilities:
* Coordinate and support Telehealth rounds in aged care facilities
* Liaise with GPs, Specialists, and on-site nursing staff to deliver seamless care
* Manage schedules, documentation, and follow-ups using smart technology
* Build strong relationships with facility staff and residents
* Support the ongoing growth of the Access Aged Care service model through collaboration and communication
Requirements:
* AHPRA registration as a Registered Nurse or Enrolled Nurse
* Minimum of 2 years experience
* Aged Care experience (leadership or clinical coordination highly regarded)
* Strong IT skills and confidence working with digital systems
* Excellent communication and relationship-building skills
* A current driver's licence and reliable vehicle
* A proactive, professional, and flexible approach to work
