Job Title: Community Care Specialist
The Community Care Specialist will support individuals living with severe mental illness and chronic physical health conditions in Wagga Wagga, NSW. This role is part of a comprehensive program aimed at promoting recovery-focused goals for participants. The specialist will coordinate care for complex needs, using a person-centred approach to develop and achieve individualized objectives.
Key Responsibilities:
* Coordinate care for individuals with complex mental health and chronic physical health needs, including assessments, goal setting, progress tracking, and planned program exits.
* Employ a holistic approach to support participants, families, and carers in developing and achieving recovery-focused goals.
* Provide education on physical health topics to consumers and the broader community, focusing on wellness and disease prevention strategies.
* Facilitate community sessions and initiatives to raise awareness about the interconnectedness of physical and mental health.
