We are seeking applications from a suitably qualified Care Partner in Port Macquarie.
About The Position
This 12‑month contract, with a view to permanency, involves managing client registration, care planning and the coordination of services to ensure the delivery of quality, client‑centred customised care services. The Community team delivers consumer‑directed care for older people including personal care, home nursing, domestic assistance and a range of other support services such as shopping, transport and respite. Services are delivered under the Home Care Package and Commonwealth Home Care Package Program.
Responsibilities
* Plan, coordinate, oversee and evaluate client packages.
* Undertake initial client intake and registration including sample budgets.
* Develop and maintain care plans and individualised budgets.
* Coordinate client services, including liaising with other services as required.
* Monitor individual programs and budgets; check and confirm service delivery data for processing.
* Manage activity finances including expense authorisations.
Qualifications and Experience
* Experience and/or strong understanding of coordinating support services for people with varying needs in aged and home care (including those with additional high or complex needs).
* Minimum qualification: Cert IV in Aged Care.
* Strong written, verbal and interpersonal communication skills.
* Strong time‑management and prioritisation skills.
* Ability to work creatively and collaboratively to problem‑solve.
* Current Driver's Licence.
* Ability to obtain a NDIS Workers Screening Check Clearance.
* A team player who is also capable of working autonomously.
* Ability to establish a connection with customers and build a solid professional relationship.
* Self‑motivation and ability to motivate others.
