Responsibilities
* Facilitate on‐ground support, implementation of relationship‐based support strategies, modelling and coaching with clients, carers and care staff.
* Collaborate with peers to develop skills and knowledge of aged care staff and carers.
* Support implementation of recommendation plans on the ground, using modelling and coaching techniques, and seek guidance from the team leader or consultants as required.
* Facilitate and support trial and implementation of brokerage items with clients and carers.
Qualifications
* Strong organisational abilities, effective time management, and ability to prioritise in a busy environment.
* Ability to work independently and as part of a team.
* Commitment to excellence in customer service principles including conflict resolution.
* Current driver's licence and safe driving record.
* Willingness to travel intrastate and interstate.
* Qualification in a relevant field (Allied Health Assistant, Diversional Therapy, Enrolled Nursing, Aged Care, Cert III or IV in Individual Support / Leisure and Health, or similar).
