Health Care Coordinator – Wagga Wagga, NSW (Wiradjuri Country)

Part of the Psychosocial Physical Health Pilot Program, supporting individuals living with severe mental illness and chronic physical health conditions. Deliver short‑term, low‑intensity, community‑based supports that promote physical wellbeing and enhance daily functioning.

What will you be doing?

- Coordinate care for individuals with complex mental health and chronic physical health needs – including initial assessments, goal setting, progress tracking, and planned program exits.
- Use a person‑centred approach to support participants, families, and carers in developing and achieving recovery‑focused goals.
- Deliver health and wellness education to consumers and the broader community, focusing on physical health topics.
- Facilitate community sessions and anti‑stigma initiatives to raise awareness among the public and health professionals about the vital connection between physical and mental health for overall wellbeing.

What do I need to bring to the role?

- Relevant registration : Registered or Enrolled Nurse with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA, or a qualified Aboriginal Health Worker.
- Robust clinical skills : Ability to conduct comprehensive assessments, develop care coordination plans, deliver safe practice, and evaluate outcomes.
- Recovery‑oriented mindset : Understanding of recovery‑based approaches and ability to work collaboratively with individuals and families.
- Knowledge of chronic care : Competencies and/or understanding of supporting individuals with chronic health conditions.
