About the Role
The Active Support Facilitator plays a pivotal role in embedding evidence-informed Active Support practices across Community Services.
Working closely with Service Managers, frontline teams, participants, and families, you will support the delivery of high-quality, person-centred supports that promote choice, control, inclusion, independence, and meaningful participation.
This role combines:
* Participant outcome assessment
* Practice coaching and mentoring
* Service quality monitoring
* Data analysis and reporting
You'll be instrumental in strengthening workforce capability while ensuring participants experience genuine engagement, autonomy, and improved quality of life.
What You'll Be Doing
Participant Outcomes & Active Support
* Conduct Canadian Occupational Performance Measure (COPM) assessments and reviews
* Support participants to identify meaningful goals aligned to their preferences and aspirations
* Develop and embed person-centred Active Support strategies
* Monitor participant progress and identify opportunities to improve engagement, wellbeing, and independence
* Promote participant-led supports aligned with NDIS principles of choice and control
Practice Quality & Workforce Coaching
* Conduct OPQ (Observing Practice Quality) observations across services
* Provide real-time coaching, mentoring, and reflective feedback to frontline employees
* Support teams to strengthen communication, autonomy, engagement, and inclusion practices
* Promote consistency and best‐practice Active Support approaches
* Assist leaders to embed reflective practice and continuous learning
* Analyse participant outcome and service quality data
* Identify trends, risks, and improvement opportunities
* Prepare reports and recommendations for leadership teams
* Contribute to continuous improvement projects and service reviews
Safeguarding & Compliance
* Promote compliance with NDIS Practice Standards and organisational policies
* Foster a culture of dignity, safeguarding, respect, and participant empowerment
* Identify and escalate risks, concerns, or practice gaps
* Support proactive strategies that reduce incidents, complaints, and restrictive practices
About You
You are a passionate and values-driven professional who genuinely believes in empowering people living with disability to live meaningful and self-directed lives.
You bring:
* Strong understanding of person-centred and Active Support practices
* Experience within disability, community services, allied health, or human services environments
* Excellent communication, coaching, and relationship-building skills
* Confidence facilitating observations, reflective practice, and difficult conversations
* Strong analytical and problem-solving abilities
* A commitment to inclusion, safeguarding, and continuous improvement
* Qualification in Developmental Education, Social Work, Allied Health, Disability, Community Services, or related field
* Experience working within the disability sector
* Knowledge of NDIS Practice Standards and Quality Indicators
* Current NDIS Worker Screening Clearance
* Current Working With Children Check (where applicable)
* Current Driver's Licence
* Current First Aid and CPR certification
* Ability to travel across service locations
* Experience using COPM or other outcome measurement tools
* Experience conducting practice observations or quality reviews
* Experience coaching, mentoring, or developing teams
* Understanding of Supported Independent Living (SIL) environments
* Experience in reporting, data analysis, or continuous improvement
