Wurli-Wurlinjang Health Service is an Aboriginal Community Controlled Organisation delivering primary and social health care services to the Aboriginal people in Katherine and surrounding communities. Wurli prides itself on delivering a range of health services to the community in a style and manner that makes clients feel welcome and comfortable. Wurli operates a range of clinics and provides a range of primary health care and other services which include main clinic (acute and general care), Gudbinji Chronic Disease Clinic, Women and Children's Health and Men's Health. Wurli is funded by the Primary Health Network NT to deliver an Integrated Team Care Program that aims to contribute to improved health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care.

ITC Program Lead

The ITC Program Lead role is responsible for identifying and facilitating improvements in ITC client care.

* A current resume or curriculum vitae (CV)
* A cover letter / email which provides a clear and concise overview of your ability to meet the key requirements of the role (prerequisites, qualifications / experience and competencies)
* Bachelor of Nursing / Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care
* Minimum five (5) years clinical experience. Previous experience in providing chronic disease clinical care
* Fully Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA) / Fully Registered as ATSI Primary Health Practitioner with Australian Health Practitioners Regulation Agency (AHPRA)
* An understanding of Aboriginal Community Controlled Health Organisations.
* National Police Records Check and Working with Children's Clearance
* Current Northern Territory Driver's License
