is pleased to invite Expressions of Interest (EOI) from suitably experienced and motivated staff for the position of Service Coordinator.
This role is an excellent opportunity for individuals who are passionate about delivering high-quality, person-centred supports and who are committed to supporting participants to achieve their goals under the NDIS framework.
Key Responsibilities include:
* Managing and maintaining accurate participant documentation and records
* Monitoring participant goals and implementing support plans
* Conducting risk assessments and ensuring safety compliance
* Coordinating rosters and ensuring effective service delivery
* Liaising with participants, families, and external stakeholders
* Ensuring compliance with organisational policies and NDIS standards
Essential Skills & Experience:
* Degree in Social Work
* Experience in disability, mental health, social work or community services (Desired)
* Strong understanding of NDIS practices and documentation requirements
* Excellent communication and stakeholder engagement skills
* Ability to implement and monitor support plans
* Ability to lead, support, and supervise a team
* Experience conducting risk assessments and managing incidents
* Experience using systems such as Brevity
