Hunter Primary Care is seeking a passionate and skilled Mental Health Clinical Care Coordinator on a part time basis (up to 26 hours per week) to join our dynamic Primary Mental Health team.
ABOUT THE ROLE
You'll provide time-limited, community-based clinical care coordination to people experiencing significant functional impairment due to mental health challenges and psychosocial complexities. This includes comprehensive assessments, evidence-based interventions, and collaborative care planning with GPs, psychiatrists, allied health professionals, and peer navigators.
You'll also contribute to the Transitional Care Packages (TCP) Program, supporting clients transitioning from Hospital to primary care
DUTIES
* Conduct clinical assessments and develop tailored care plans
* Deliver evidence-informed interventions and coordinate care across services
* Facilitate access to psychosocial supports and lifestyle interventions
* Collaborate with families, carers, and multidisciplinary teams
* Provide assertive outreach and post-discharge support
* Work closely with Peer Navigators in culturally appropriate care delivery
* Maintain accurate clinical records and contribute to service development
SKILLS & EXPERIENCE
* Degree qualifications in a mental health profession
* Current AHPRA registration (as appropriate) and/or Accreditation with the appropriate professional body (AASW etc)
* Mental Health Accreditation (for nursing, social work, occupational therapy), or working towards same.
* Minimum of three years' clinical experience
* Demonstrated experience in delivery of community based mental health services including care coordination, crisis intervention and/or assertive community treatment
* Demonstrated ability to establish and maintain effective relationships with key stakeholders
* Highly developed written and verbal communication skills including the ability to engage with a diverse range of people.
* Demonstrated capacity to work autonomously within a strong clinical governance framework.
* Skilled in the use of computers/clinical software including client management systems and Microsoft Office programs.
* Vaccinations as required in line with Hunter Primary Care policies
* Current NSW Working with Children Check clearance
