About The Role
The Clinical Intake Worker supports access, coordination, and the client journey across community and non‐admitted Allied Health and Ambulatory Care services at Grampians Health Stawell. This role is part of a central intake team, working alongside administration service access staff to ensure timely, appropriate, and coordinated access to care.
Key Responsibilities
* Acting as an initial point of contact for clients
* Patient registration and information gathering
* Initial needs identification
* Clinical screening and clinical triage
* Liaising with internal and external providers
The successful applicant will have excellent communication skills and a strong commitment to supporting patients to access the right care, at the right time, in the right location.
You Will Bring
* Strong skills in medical terminology
* Knowledge of public health pathways and local agencies
* The ability to use clear, lay language to sensitively ask prompting questions about a person's health and social needs when accessing services
