About the Role
You will play a vital role in helping residents maintain their homes and live more independently. Through early intervention, person-centred planning, and cross-sector collaboration, you'll help prevent homelessness and build pathways to recovery.
This role is ideal for someone who is a natural connector, has a calm presence in complexity, and is passionate about social impact.
* Deliver leading tenancy sustainment solutions with a trauma-informed approach
* Support up to 25 residents through person-centred support plans and collaborative engagement
* Identify tenancy risks and work proactively with Housing Officers to prevent tenancy breakdown
* Work alongside mental health specialists, NDIS providers, and community agencies
* Champion best practices, lead service improvements, and contribute to program evaluation
* Be a trusted advisor to internal teams, sector partners, and support networks

Desired Skills and Experience
You will have:
* Relevant qualifications in community services, social work, or mental health
* Proven experience in case management or tenancy support for people with complex needs
* Ability to manage conflict, navigate challenging behaviours, and build trusted relationships
* Knowledge in areas such as mental health, AOD, trauma, disability, ageing, or cultural safety
* Exceptional communication, planning, and stakeholder engagement skills
* Will require travel to the Greater South West Region
