Supported Living Facilitator | Disability Services, NDIS

Employment Term: Casual
Location: Toowoomba
Remuneration: $43.23 p/hr + Super + NFP Salary Packaging

Responsibilities
* Work in partnership with the person being supported, their family and informal support networks, to attain the hopes and aspirations they have established for their lives
* Provide support to people in their everyday living situations, including personal care as identified, with an emphasis on choice, enhancing quality of life
* Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people
* Enable people to attain personal autonomy over their own lives and the services which they receive
* Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person being supported and their family
Qualifications
* A relevant industry qualification (e.g., Certificate III in Disability or Individual Support) is desirable
* Experience working in the Disability, Aged and Community sector is ideal but not essential – those with a positive attitude and willingness to learn are encouraged to apply
* Current NDIS Worker Screening check and Blue Card (or in the process of obtaining)
* Current open driver's licence
* Current CPR and First Aid Certificate (required before start)
* Ability to work effectively with others in a team‐oriented environment and provide excellent support to clients and co‐workers
* Flexibility (able to work varied hours and days) and reliability to meet the needs of the people we support
* Effective communication in spoken and written English
