Key Responsibilities
* Coordinate the provision of patient care services, ensuring effective communication among team members and stakeholders.
* Assess and analyze patients' health status, identifying areas where interventions can improve their quality of life.
* Develop and implement individualized care plans that address the physical, emotional, and social needs of residents.
* Maintain accurate and up-to-date records of patient interactions, progress notes, and treatment plans.
Requirements
* Bachelor's degree in nursing or a related field.
* Possession of current registration as an enrolled nurse or registered nurse.
* At least 2 years of experience in aged care, preferably in a leadership or coordination role.
* Strong organizational, communication, and interpersonal skills.
