Clinical Nurse Consultant – Transition Coordination
Join the Queensland Spinal Cord Injuries Service (QSCIS) at the Princess Alexandra Hospital as a Clinical Nurse Consultant – Transition Coordination. In this advanced leadership role, you will provide expert clinical guidance and coordination across the continuum of spinal injury care.
Responsibilities
* Deliver advanced clinical leadership and consultation in spinal cord injury rehabilitation and discharge planning.
* Provide specialist advice and coordinate complex patient transitions across hospital and community care.
* Support the development of clinical standards, protocols and evidence‐based practice for spinal rehabilitation.
* Mentor and guide nursing and multidisciplinary colleagues to promote excellence and consistency in patient care.
* Lead quality improvement and clinical redesign initiatives to enhance service delivery and patient outcomes.
* Contribute to research, education and innovation within the QSCIS framework.
About You
You're an experienced and motivated nurse leader with advanced clinical expertise and a passion for improving outcomes for people with spinal cord injuries.
Qualifications
* Extensive experience in spinal rehabilitation, discharge planning or complex care coordination.
* Strong leadership and mentoring skills to support staff development and build high‐performing teams.
* Advanced clinical reasoning and problem‐solving ability in complex patient care settings.
* Commitment to continuous improvement, research and evidence‐based practice in nursing.
* Excellent communication, negotiation and collaboration skills to foster multidisciplinary teamwork.
* Postgraduate qualifications in a relevant area (or working towards one) – highly desirable.
